How We Manage Donors and Fecal Material

    Donor requirement

    • healthy from birth
    • no pathologies
    • between 1 and 6 years old
    • regularly vaccinated
    • no antibiotic use in the last 6 months, maximum one short antibiotic course in their lifetime
    • no use of other medications
    • no acute gastrointestinal episodes in the last 6 months
    • no chronic gastrointestinal problems, atopic dermatitis, or abnormal behavior

    Eligibility Evaluation

    The eligibility evaluation process includes the following steps:
    1. owner questionnaire
    2. microbiome analysis (must be compatible with the healthy reference population)
    3. stool analysis
    4. blood analysis
    5. medical examination

    Maintaining Eligibility

    Maintaining eligibility is an ongoing process throughout the period.
    The owner monitors the pet's health and fills out an online questionnaire weekly, and every month and a half, we reassess the stool. Any anomaly leads to the suspension of eligibility and the elimination of all collected material.

    Material Management

    The collected material is managed according to literature protocols for optimal preservation of microbial viability, following best practices for handling biological material, with due precautions to ensure the safety of both the material and the handler.
    Once it reaches the laboratory, it is pre-processed (dilution, filtration, addition of cryoprotectant) under a biological safety cabinet and quarantined at -80°C.
    Only if repeated analyses after the collection period show optimal results will the material be released, freeze-dried, and encapsulated. The powder is characterized with NGS-16S sequencing both as a quality control and for subsequent evaluations of donor microbiome engraftment.

    FAQ

    • Who are the recipients?
      Dogs and cats with symptoms of chronic enteropathy for at least 3 weeks and atopic dermatitis.
    • Are there any exclusions?
      • Enteropathies due to parasitosis, neoplasms, endocrinopathies, dysphagia, or age under 4 months.
      • Dermatitis caused by parasites.
    • Is FMT the last resort?
      No!
      The logic is that fecal transplantation aims to rebalance the microbiome, but if organs and tissues are severely compromised, FMT is unlikely to make a significant difference.
    • When to do FMT then?
      More and more veterinarians suggest using fecal transplantation after the failure of dietary trials, possibly accompanied by drugs that reduce the inflammatory state. The sooner you intervene when dysbiosis is suspected, the better.
    • Is it necessary to stop other therapies?
      Fecal transplantation is a treatment that can be combined with other therapeutic approaches available to the veterinarian, who is solely responsible for the therapy. We only suggest not using antibiotics and probiotics simultaneously, based on the logic that they may be potential competitors to the transplant itself.
    • What does the FMT kit contain?

    • How are the capsules administered?
      One capsule a day orally, with a tidbit if needed.
    • Can the administration be stopped?
      Yes, you can suspend and resume later. The goal of transplantation in chronic enteropaths is to rebalance the microbiome, which must be accompanied by a general rebalancing of the organism, therefore the transplant must be seen in a long-term perspective.
    • How are the capsules stored?
      At room temperature protected from light and humidity.
    • Are there any side effects?
      Rarely diarrhea, vomiting, flatulence, abdominal pain that resolve on their own in 24-48 hours, in case suspend for 3-4 days.
    • Is one treatment enough?
      It depends on the case, there are situations in which there is an improvement immediately, others in which it arrives even after the end of the transplant; the improvement may be definitive or there may be relapses that require more cycles to arrive at a stable resolution of symptoms. In some cases, no improvement in symptoms is seen. Today no one can say a priori how long it will take, how many treatments will be needed, but a light can be glimpsed (see the paper on ulcerative colitis in humans in which a treatment with capsules has been done for 56 consecutive weeks), and the Pet FMT Project can make its contribution.
